The information you are about to complete is a requirement for entry into Canada and or re-entry into the United States.

Please complete the following information. You will find the group name and invoice number on your invoice in the top right corner. If you have received a email confirmation, the group name and invoice number will be in the top right hand corner also. The invoice confirmation number is the most important part! (example: 4184) All fields are required fields to be completed. Please complete the names and numbers as they read on the passport ONLY.

You are required to complete all fields.
Non-US Citizens:
Please call to submit your passenger information to 1 800 690-0086 ext. 2. Since there are additional requirements for Customs Clearance for NON-US Citizens, we will require additional information from you. Please have the address which you will be staying in United States.
